A large federal agency identified a systemic FLSA miscalculation affecting hundreds of employees. The error had gone unresolved and liability was growing.

 

4T stepped in as operational lead — supervising the full calculation and manual pay correction process from intake through resolution. Every action documented to audit standard. Coordination managed directly with NFC and HRIS.

4T initiated a full return-to-work effort — collaborating with reasonable accommodations, establishing a tracking report, and creating bi-annual executive briefings with the Chief Administrative Officer. The program was restructured from reactive to managed.

Case Study 03 — Enterprise HR Records Audit

3,600 Records Audited.
71 Retirement Errors Corrected. 9,000 More Under Review.

A federal agency with a workforce of over 13,000 employees had no enterprise-wide audit process for verifying HR record accuracy. Retirement coverage codes, SCDs, FEGLI designations, and TSP enrollments were processed across divisions — but never systematically verified.

 

4T designed and initiated an agency-wide audit examining SCDs, retirement codes, tenure, pay, and benefits across the full workforce. Cross-divisional corrective action was coordinated across employment, systems, and resource planning. A quality review framework was established that did not exist before the engagement began.

The Result:

  • 3,600 records reviewed

  • 12 FERCCA errors and 59 retirement coverage errors corrected

  • 9,000 additional cases initiated for review

  • Quality review framework built and left in place
